                                                                                                                                                This document includes instructions for if your child has symptoms, close contact with a positive COVID-19 case or travelled outside of Canada.
                                                                                                                                                  The COVID-19 protocols have been adapted from the Ontario Ministry of Education and York Region Public Health Guidelines and are subject to change.
                                                                                                                                                Voluntary Student Vaccination Disclosure

                                                                                                                                                Currently, the York Region and Government of Ontario COVID-19 School Protocol states that if a child is symptomatic their siblings may continue to attend PCA if they exhibit no symptoms and are fully vaccinated. For this reason, if you are comfortable sharing your child’s COVID-19 proof of vaccination, please upload their second dose vaccination proof to https://forms.gle/4kc9BVHCNz6pcspL6.

I completely respect and understand that not everyone is comfortable sharing their vaccination status and that not all children are able to be vaccinated. If I do not receive proof of vaccination, then your child will simply follow the “unvaccinated protocol” by which all children, both ill and not ill within the same household, will stay home together.

                                                                                                                                                 PCA Pre-Existing Medical Condition Form
                                                                                                                                                     If your child has any preexisting medical conditions please fill in the Pre-Existing Medical Condition Form found here and email it to office@pca.ca.                                                                                                                                                                                                                                                     A pre-existing medical condition is considered a medical condition your doctor has diagnosed your child with that would cause your child to exhibit symptoms found on the daily screening. For example, seasonal allergies may cause your child to have a runny nose or asthma may cause your child to have shortness of breath.  When your child exhibits symptoms that are found on the daily screening and PCA has your Pre-Existing Medical Condition Form on record, your child may follow the Pre-Existing Medical Condition section found on the PCA COVID-19 Protocol.

Complete one form at https://forms.gle/fKivUXfi5Q6fn3Wd6 every morning for each child that will be attending PCA. A copy of the results will be reviewed before entry into PCA is permitted. Please take a print screen on your phone or a printout of the green figure, student’s full name and today’s date. The screening questionnaire has been adapted from the Ontario Ministry of Education and York Region Public Health Guidelines and is subject to change.

 

PCA Absence Form 

Please complete and submit this form at https://forms.gle/NEzjwCv7sgXufYPb8 if your child will be absent for:

  • one or numerous days (regardless if they will be attending online or not)

  • arriving late on a specific day

  • dismissed early on a specific day

For children that have selected the FULLY IN-PERSON option this school year, please fill in this form if your child will not be attending PCA in person, regardless if your child will be present online or not.

                                                                                                                                                  Do not complete this form if your child has received special permission to temporarily switch online for a short period of time.
                                                                                                                                                  Please note, the parent or guardian must fill in this form, not the child.

 

PCA Transition to Online or in-Person Form

Please complete and submit this form at https://forms.gle/bCgNpzknez4f4WWU9 if your family has made the decision to conveniently transition your child to online or in-person from their previous status.  Please note, transitions will be effective on the date selected, not on the date of form submission.

mmunization Disclosure Policy 

All staff and volunteers must provide one of the following:
1. proof of all required doses of a COVID-19 vaccine approved by the World Health Organization
2. proof of completion of an education program and COVID-19 rapid antigen testing twice weekly
                                                                                                                                                  Additionally, all staff rapid testing will occur a minimum of once a month. Please note all personal and private medical information provided by staff, students and parents will remain strictly confidential and will not be disclosed to anyone.

If your child is fully in person: your order will be distributed to your eldest child.

If your child is fully online:

  1. Please arrive at door 1 Monday to Fridays between 10 am to 2 pm
  2. Buzz the office stating you are here for face mask pick up and your eldest child’s full name and homeroom
