PCA COVID-19 Protocol

If anyone in your household is experiencing symptoms, had close contact with a positive COVID-19 case or travelled outside of Canada please keep your family at home and contact PHCO at for further guidance. The COVID-19 protocols have been adapted from the Ontario Ministry of Education and York Region Public Health Guidelines and are subject to change. PCA Pre-Existing Medical Condition Form
If your child has any preexisting medical conditions please fill in the Pre-Existing Medical Condition Form found here and email it to  A pre-existing medical condition is considered a medical condition your doctor has diagnosed your child with that would cause your child to exhibit symptoms found on the daily screening. For example, seasonal allergies may cause your child to have a runny nose or asthma may cause your child to have shortness of breath. When your child exhibits symptoms that are found on the daily screening and PCA has your Pre-Existing Medical Condition Form on record, your child may follow the Pre-Existing Medical Condition section found on the PCA COVID-19 Protocol.


PCA Daily Screening Link

Complete one form at every morning for each child that will be attending PCA. A copy of the results will be reviewed before entry into PCA is permitted. Please take a print screen on your phone or a printout of the green figure, student’s full name and today’s date. The screening questionnaire has been adapted from the Ontario Ministry of Education and York Region Public Health Guidelines and is subject to change.


PCA Absence Form 

Please complete and submit this form at if your child will be absent for one day, arriving late on a specific day, dismissed early on a specific day or absent for numerous days from school.  An absence is considered not being present in person or online. Please note, the parent or guardian must fill in this form, not the child.


PCA Transition to Online or in-Person Form

Please complete and submit this form at if your family has made the decision to conveniently transition your child to online or in-person from their previous status.  Please note, transitions will be effective on the date selected, not on the date of form submission.

PCA Immunization Disclosure Policy 

All staff and volunteers must provide one of the following:
1. proof of all required doses of a COVID-19 vaccine approved by the World Health Organization
2. proof of completion of an education program and COVID-19 rapid antigen testing twice weekly
Additionally, all staff rapid testing will occur a minimum of once a month. Please note all personal and private medical information provided by staff, students and parents will remain strictly confidential and will not be disclosed to anyone.

If your child is fully online:

  1. Please arrive at door 1 Monday to Fridays between 10 am to 2 pm
  2. Buzz the office stating you are here for face mask pick up and your eldest child’s full name and homeroom
